Scholarships
The Lyme Disease Association, Inc. (LDA) is offering scholarships to the conference to eligible medical students, residents, post-doctoral candidates, fellows, veterinarians with equivalent status to the above, and nurse practitioner candidates. Click here for Scholarship Application.

General Information on Registration
► Fees:
• Regular – $215 (through 09/11/16); $235 (beginning 09/12/16); $255 (if space available, at door)
• CME – $375 (through 09/11/16; $395 (beginning 09/12/16; $420 (if space available, at door)
► The availability of last minute “at the door” registrations is determined by the room capacity and number of registrants on day of conference. Please note, the rate to register “at the door” will be higher than the pre-registration fee. There is a CASH or CHECK ONLY policy when registering “at the door”.
► No phone or fax registrations accepted
► CANCELLATION: Written notice of cancellation must be received by LDA by 09/6/16 for refund. Refund by that date will be equal to 90% of the registration price paid. NO cancellations will be accepted after that date. NO monies will be returned. NO exceptions. Email cancellation notice to [email protected] or fax 732-938-7215. Deadlines are based on Eastern Time.
► AVAILABILITY: Registration subject to space availability at the time of receipt by LDA of completed registration. NO registrations accepted without payment at time of receipt.
Attention: Minnesota Public Health Officials
The Minnesota Lyme Association (MLA) is offering to sponsor the attendance of MN public health officials at the conference. Specifically, MLA will reimburse the registration fee of one staff member from each county in MN who is working on vector-borne diseases. To qualify, attendees must register before Oct 1, 2016 and attend both days of the conference. Potential reciepents must receive approval from the Minnesota Lyme Association prior to registering. Poster Presentation Information
Conference poster submissions: please email the copy to [email protected] by September 12, 2016 for review and determination of acceptance. All decisions by the review committee are final. Poster presenters must be registered conference attendees.

Accreditation: This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the College of Physicians and Surgeons of Columbia University and the Lyme Disease Association. The College of Physicians and Surgeons of Columbia University is accredited by the ACCME to provide continuing medical education for physicians.
AMA Credit Designation Statement: The College of Physicians and Surgeons designates this live activity for a maximum 14 AMA PRA Category 1 Credits TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Target Audience: The target population is physicians from all specialties, nurses, psychologists, scientists, public health workers. It is also open to the public, and Lyme disease educators generally attend. The geographic area being reached is nationwide. No special background is required for effective participation, although those whose practices contain a high proportion of Lyme disease patients and those whose research concentrates on Borrelia burgdorferi will receive the most benefit.
Learning Objectives: Practitioner should be better able to identify the multiple manifestations of Lyme disease. Practitioner should be better informed about new diagnostic advances. Practitioner should be able to conduct a broader differential diagnosis when faced with a patient with illness after tick bite. Awareness that there is a new mouse vaccine that helps diminish tick infection rates.
